Can they also appear in other areas?

Xanthelasmas are yellowish or orange-colored skin lesions that resemble plaques or patches. They usually appear around the eyes and have clearly defined borders.

Very rarely, cholesterol plaques or yellow patches may also appear on other areas of the body, such as the hands, elbows, knees, labia in women or the foreskin of the penis in men.

At what ages do eyelid xanthelasmas appear and how common are they?

Eyelid xanthelasmas appear in men and women between the ages of 40 and 60. It is extremely rare for them to develop in adolescents, young adults or very elderly individuals.

Although this condition affects both sexes, women appear to develop xanthelasmas almost twice as frequently as men.

Are eyelid xanthelasmas dangerous?

Xanthelasmas themselves are completely harmless lesions from a medical perspective. The problem they cause is primarily aesthetic rather than functional, since they never affect vision and never develop into malignancy.

Studies have shown that up to 55% of individuals with xanthelasmas may also have lipid metabolism disorders, resulting in high cholesterol levels in the blood. Only in these cases should hypercholesterolemia be treated in order to prevent possible cardiovascular complications in the future.

What is the relationship between hypercholesterolemia, meaning high blood cholesterol levels, and the appearance of eyelid xanthelasmas?

There is no absolute or direct correlation between hypercholesterolemia and the appearance of eyelid xanthelasmas.
In other words, having xanthelasmas does not necessarily mean that I also have high cholesterol, nor does having high cholesterol necessarily mean that I will develop xanthelasmas.

If I take medication for cholesterol, can I prevent xanthelasmas from developing?

The appearance of xanthelasmas is not directly related to blood cholesterol levels. According to current studies, taking lipid-lowering medication neither helps nor prevents the development or expansion of existing xanthelasmas.

I have xanthelasmas on my eyelids. Should I undergo general laboratory tests or blood tests?

Given that up to 55% of xanthelasmas appear in individuals with abnormal lipid profiles, we recommend that patients with xanthelasmas undergo periodic testing of their lipids, lipoproteins and cholesterol levels.

These tests do not help us monitor or treat the xanthelasmas themselves, but they help identify and subsequently correct an underlying subclinical hyperlipidemia.

Is there a chance that xanthelasmas will decrease in size over time?

Once xanthelasmas appear on the skin, they are permanent. They never decrease in size. In fact, the opposite is true. If they are not treated promptly, they tend to increase in both size and number and to spread locally.

Is there a hereditary predisposition to the development of xanthelasmas?

Eyelid xanthelasmas are often observed in members of the same family, suggesting the existence of a hereditary predisposition.
In these cases, hereditary disorders affecting lipid metabolism often coexist, resulting in high blood cholesterol levels, as in familial hypercholesterolemia.

What is the treatment for xanthelasmas?

Xanthelasmas can essentially be treated in two ways:

by destroying the cholesterol plaques using cryotherapy with liquid nitrogen or a CO2 Laser, or

by completely removing the cholesterol plaques using Plastic Surgery techniques.

How are xanthelasmas treated with cryotherapy or CO2 Laser?

The procedure is usually performed under local anesthesia. Liquid nitrogen is applied to freeze the xanthelasma, or a carbon dioxide Laser is used to burn the lesion. The destruction of the cholesterol plaque creates a wound in the area, usually followed by a recovery period that may be uncomfortable for the patient and requires care with frequent dressing changes.

Because the destruction of xanthelasmas is not completely controlled, is associated with a longer recovery period and has higher recurrence rates, we generally recommend and follow surgical removal as the preferred treatment for xanthelasmas.

How are xanthelasmas removed using Plastic Surgery techniques?

Our objective during the surgical treatment of xanthelasmas is not to destroy the cholesterol molecules within the plaques, but to remove them completely.
By surgically removing the cholesterol plaques, we eliminate the areas of lipid deposition, resulting in considerably lower recurrence rates compared with other techniques.

What is the procedure for the surgical removal of xanthelasmas?

The procedure begins with careful planning of the xanthelasmas so that the cholesterol plaques can be removed completely, while preserving the aesthetic harmony of the area around the eyes and the full functionality of the upper and lower eyelids.

The procedure is highly controlled and is performed under local anesthesia in an organized surgical environment.

After the xanthelasmas have been removed, extremely fine sutures are placed in the surgical area and the incisions are covered with very thin adhesive strips.

Is xanthelasma removal painful? What will I be like immediately after surgery?

Although the area around the eyes is very sensitive, it is remarkable that xanthelasma removal is not painful either during the procedure or during the postoperative period. The only sensation usually experienced is pressure or mild discomfort, since the area develops very mild swelling immediately after surgery.

The typical postoperative appearance includes slight swelling and occasionally small bruises around the eyelids.

The swelling and bruising are more pronounced during the first two to three days, while by the end of the week they have almost completely subsided.

What should I be careful about and what should I avoid after the procedure?

Immediately after the procedure, the patient may leave the clinic and return home with instructions.
Because we do not cover the eyes and only cover the incisions with very thin adhesive strips, the patient can go out immediately after surgery.

During the first two to three days after the procedure, we ensure that the incisions remain clean and sterile. This is achieved by alternating ophthalmic ointments and eye drops, which can help prevent infections.
Mild swelling or a small bruise may develop around the eyes, but these usually subside within the following 3 to 5 days and rarely cause discomfort to the patient.

Immediately after surgery and for the following 5-7 days, the patient should avoid bending over and intense physical activity, particularly activities that increase blood pressure, such as weightlifting and sports, in order to prevent the formation of a hematoma or other complications.

Patients who wear contact lenses should avoid wearing them until the eye area has completely settled, meaning for approximately 2 to 3 weeks. They may watch television or read after one or two days and can usually return to work two to three days after the procedure.

Regarding suture removal, we usually use absorbable sutures that do not need to be removed, as they typically fall out on their own after approximately 10 to 15 days.

What complications may occur after xanthelasma removal?

Because the tissues around the eyes are very delicate, the most common events after xanthelasma removal are bruising and swelling, which gradually disappear within 5-7 days.
Photophobia, excessive tearing or reduced tear production are extremely rare after xanthelasma removal. They are treated with artificial tears and by avoiding exposure to bright light.
Hematoma, inflammation or infection of the area are also rare complications and are treated with drainage and antibiotic therapy respectively.

If xanthelasmas are removed, can they develop again after some time?

When xanthelasmas are treated by destroying them using cryotherapy or Laser, the recurrence rate is approximately 15 to 20%. The reason is that with these techniques we cannot be completely certain that all cholesterol molecules have been destroyed. Any remaining molecules may act as focal points for further lipid deposition within the cholesterol plaques.

By applying the surgical-removal method that we recommend, the area can be thoroughly examined during the procedure and all lipid plaques can be removed deeply and within healthy tissue margins. In this way, it is highly unlikely that cholesterol molecules will remain in the area.

With surgical removal of xanthelasmas, the recurrence rate is very low and ranges from 1 to 5%.

The procedure is very well tolerated by the patient.
