LIPOMA<\/h2>\n\n\n\n

Lipomas are lumps that develop in the subcutaneous tissue, meaning just beneath the skin. They usually look like swellings that deform the appearance of the area where they appear and, when they grow significantly, give it a particularly unsightly or even unusual appearance.<\/p>\n\n\n\n

What are Lipomas made of?<\/h2>\n\n\n\n

They are lumps made up of organized fat cells, which behave differently from the fatty tissue found in the subcutaneous layer, since:<\/p>\n\n\n\n

1st. they are often well-circumscribed tumors, since they are surrounded by a firm membrane, or capsule, and become autonomous within the fatty tissue, and<\/strong><\/p>\n\n\n\n

2nd. they tend to gradually increase in size regardless of the person\u2019s weight gain or weight loss.<\/strong><\/p>\n\n\n\n

Lipomas may appear either as a single swelling of the skin in one area of the body or may be multiple and develop scattered in many different locations. They may also be unilocular or multilocular, meaning lobulated.<\/p>\n\n\n\n

How common are Lipomas in the human body?<\/h2>\n\n\n\n

Lipomas are the most common benign tumors that appear in the soft tissues and skin of the human body. Specifically, they develop in approximately 1% of people, and their occurrence is clearly more common in women than in men.
They are usually not present at birth, nor are they detected at young ages.
The most common age of appearance is between 40 and 60 years. In some cases, however, they may begin to develop very slowly starting around the age of 30.<\/p>\n\n\n\n

What are the most common areas where Lipomas appear?<\/h2>\n\n\n\n

In general, lipomas may appear anywhere in the subcutaneous tissue of the body. However, the most common areas where they appear are:<\/p>\n\n\n\n

    \n
  1. the forehead<\/strong><\/li>\n\n\n\n
  2. the nape<\/strong><\/li>\n\n\n\n
  3. the neck<\/strong><\/li>\n\n\n\n
  4. the shoulders<\/strong><\/li>\n\n\n\n
  5. the back<\/strong><\/li>\n\n\n\n
  6. the chest<\/strong><\/li>\n\n\n\n
  7. the sides of the abdomen, and<\/strong><\/li>\n\n\n\n
  8. the upper and lower limbs.<\/strong><\/li>\n<\/ol>\n\n\n\n

    Are Lipomas dangerous? How much should we worry if they appear on our body?<\/h2>\n\n\n\n

    Lipomas are benign tumors, meaning they are not dangerous from an oncological point of view.
    The characteristics presented by lipomas are those that distinguish all benign tumors. Specifically, lipomas:<\/p>\n\n\n\n

      \n
    1. grow very slowly and at a steady rate<\/strong><\/li>\n\n\n\n
    2. have a smooth shape, not an irregular one<\/strong><\/li>\n\n\n\n
    3. are solid but have a soft texture<\/strong><\/li>\n\n\n\n
    4. are located under the skin and are often easily palpable<\/strong><\/li>\n\n\n\n
    5. are mobile and do not firmly adhere to the underlying tissues<\/strong><\/li>\n\n\n\n
    6. do not cause itching<\/strong><\/li>\n\n\n\n
    7. do not bleed<\/strong><\/li>\n\n\n\n
    8. do not ulcerate<\/strong><\/li>\n\n\n\n
    9. are predominantly painless tumors<\/strong><\/li>\n\n\n\n
    10. NEVER metastasize, except in the case of liposarcomas.<\/strong><\/li>\n<\/ol>\n\n\n\n

      The problem they cause is mostly aesthetic rather than functional, since they almost never transform into malignancy.
      To be precise, we should note that the only malignant transformation lipomas may undergo is into liposarcomas, something that is extremely rare.<\/p>\n\n\n\n

      When is it recommended to remove Lipomas?<\/h2>\n\n\n\n

      Most of the time, we remove lipomas for aesthetic reasons and less often for pathological reasons.
      As mentioned, lipomas are generally painless tumors and, when they are small, they are usually not easily noticeable except through very careful observation. Also, most lipomas grow very slowly, and often many of them eventually stop growing completely.
      Therefore, if they do not bother the patient, do not hurt and grow very slowly, there is no cause for concern and no reason for their surgical removal.
      If they grow rapidly, if they are located on flexion or extension joint surfaces and are painful, then we recommend their surgical removal.<\/p>\n\n\n\n

      Is the development of lipomas related to obesity or hypercholesterolemia?<\/h2>\n\n\n\n

      No, the development of lipomas is not related to weight gain or hyperlipidemia. Also, apart from surgical excision, there is no safe pharmaceutical treatment for lipomas.<\/p>\n\n\n\n

      Do I need to have general laboratory tests or blood tests if a Lipoma appears on my body?<\/h2>\n\n\n\n

      No, general tests are not needed in the case of a Lipoma.
      The diagnosis of lipomas is usually made relatively easily based on the patient\u2019s history and clinical examination.
      If the appearance of the lump is unclear and there is some diagnostic doubt, then the differential diagnosis of lipomas in relation to other skin lumps may be performed with an ultrasound examination or MRI scan.
      These examinations also help us map the area and clearly see the relationship of the lipomas with the deeper tissues.<\/p>\n\n\n\n

      Is there a chance that Lipomas may decrease in size over time?<\/h2>\n\n\n\n

      Once Lipomas appear in the skin, they are permanent. They never regress in size. Usually, they remain stable or grow very slowly.<\/p>\n\n\n\n

      Why do Lipomas appear on the body? Is there perhaps a hereditary predisposition to the development of Lipomas?<\/h2>\n\n\n\n

      There is currently no definite explanation for how or why Lipomas develop.
      There is often a hereditary predisposition, since they are more common among members of the same family.
      Some individuals also report the development of a Lipoma after trauma to the area.
      However, there is no clear explanation.<\/p>\n\n\n\n

      What is the treatment for Lipomas?<\/h2>\n\n\n\n

      The treatment of Lipomas is primarily surgical. It is an oncological procedure, meaning a procedure that aims at the complete removal of the tumor, not its fragmentation or partial excision.<\/p>\n\n\n\n

      If the removal of the lipoma is radical, meaning it is performed \u201cwithin healthy tissue margins\u201d, then the recurrence rate is practically zero. On the contrary, if cells or part of the lipoma remain and are not removed, then the possibility of the lipoma recurring is very high.<\/strong><\/p>\n\n\n\n

      \"\"<\/figure>\n\n\n\n

      Given that lipomas grow very slowly, in the case of incorrectly performed partial removal of a lipoma, its recurrence usually occurs after 3 to 5 years.<\/strong><\/p>\n\n\n\n

      What type of anesthesia is used during Lipoma removal?<\/h2>\n\n\n\n

      Lipomas are predominantly superficial tumors.
      Most of the time, by applying local anesthesia, or a nerve block, we manage to anesthetize the entire area where the lipoma is located and remove it with great safety \u201cwithin healthy tissue margins\u201d, meaning 100%.
      We use general anesthesia in cases of:<\/p>\n\n\n\n

        \n
      1. large lipomas,<\/li>\n\n\n\n
      2. lipomas that extend between muscles, such as paraspinal lipomas, or<\/li>\n\n\n\n
      3. lipomas located in areas with sensitive nerve or vascular structures, such as the nape.<\/li>\n<\/ol>\n\n\n\n

        Whether with local anesthesia, neuroleptanalgesia or general anesthesia, the procedure is very well controlled when performed in an organized surgical environment.<\/p>\n\n\n\n

        What is the procedure for the surgical removal of Lipomas?<\/h2>\n\n\n\n

        The procedure begins with careful planning of the area where the lipoma appears, always respecting tissue anatomy and protecting neighboring important structures, such as vessels, nerves and ligaments.<\/p>\n\n\n\n

        After the lipoma is removed, a thorough examination of the area is performed for satellite lumps, and then the wound is closed in anatomical layers.<\/p>\n\n\n\n

        If the lump is large, drainage may need to be placed, which is usually removed after 24 hours.<\/p>\n\n\n\n

        Externally, very fine sutures are placed on the skin and the incisions are covered with special adhesive strips.<\/p>\n\n\n\n

        What do we do with the lump that we removed?<\/h2>\n\n\n\n

        The histological specimen is always sent for biopsy.<\/strong>
        In this way, we are diagnostically very safe, since we have the histopathological confirmation of the identity of the lump we removed.<\/strong><\/p>\n\n\n\n

        Is Lipoma removal painful? How will I be immediately after surgery?<\/h2>\n\n\n\n

        Given that most lipomas are located very superficially, the lipoma removal procedure generally does not hurt, either during the procedure or during the postoperative period. What patients usually describe is a feeling of pressure or mild discomfort, since the area develops moderate swelling immediately after surgery.<\/p>\n\n\n\n

        If the lipoma is very large or extends between muscle groups, then there may be discomfort or even pain postoperatively, but this is controlled with mild painkillers.<\/p>\n\n\n\n

        What should I pay attention to and what should I avoid after the procedure?<\/h2>\n\n\n\n

        Immediately after the procedure, the patient can leave the clinic and go home with instructions.
        A mild swelling or small bruise may develop in the surgical area, but these subside immediately within the next 3 to 5 days and rarely bother the patient.<\/p>\n\n\n\n

        Immediately postoperatively and for the next 2-3 days, the patient should avoid intense activity, mainly activities that increase a person\u2019s blood pressure, such as weightlifting and sports activities, in order to avoid the formation of hematoma, etc.<\/p>\n\n\n\n

        Since the surgical incision is covered postoperatively with special adhesive strips, or steri-strips, which are waterproof, the patient may bathe or allow water to fall on the surgical area without any problem.<\/p>\n\n\n\n

        Regarding suture removal, we often use absorbable sutures that do not need to be removed because they usually fall out on their own in approximately 10 to 15 days.
        Otherwise, the sutures are removed on the 6th or 7th day, and then we protect the incision for another 7-10 days with special adhesive strips, or steri-strips.<\/p>\n\n\n\n

        What complications may occur after Lipoma removal?<\/h2>\n\n\n\n

        The most common events after Lipoma removal are bruising and swelling<\/strong>, which gradually disappear within 5-7 days.
        Hematoma and inflammation or infection of the area<\/strong> are also rare complications, which are treated with drainage and antibiotic coverage respectively.
        After the removal of large lipomas, there may be the development of a seroma<\/strong>, meaning a collection of fluid in the surgical area as a result of lymphatic stasis. The fluid collection may last approximately 7 to 10 days and is more of an expected event than a complication. It is treated simply by draining the lymphatic fluid.
        Skin injury<\/strong> is generally not very common and is treated with local care.<\/p>\n\n\n\n

        Can Lipoma removal be performed with Liposuction?<\/h2>\n\n\n\n

        The removal of a lipoma with Liposuction is generally not a safe option and we do not recommend it.
        The explanation is that when we perform Liposuction, because the procedure is \u201cblind\u201d, meaning we cannot see under the skin:<\/strong><\/p>\n\n\n\n

        1st. we cannot be sure that we have removed 100% of the entire lipoma mass, so the recurrence rate is usually very high, and<\/strong><\/p>\n\n\n\n

        2nd. there is a possibility of spreading lipoma fat cells into the neighboring tissues and creating a problem ourselves in the area from which we removed the lipoma.<\/strong><\/p>\n\n\n\n

        If Lipomas are removed, can they develop again after some time?<\/h2>\n\n\n\n

        By applying the method we recommend for the surgical removal of Lipomas, during the procedure there is always careful and thorough examination, and the entire lipoma mass is removed deeply and within healthy margins. In this way, the chance of lipoma recurrence is almost zero.<\/p>\n\n\n\n

        Recurrences occur when the surgeon has performed partial removal of the lipoma, when liposuction has been performed, or when there is a small satellite lump that was not recognized and removed during the initial procedure.<\/p>\n\n\n\n

        Therefore, very careful planning and meticulous execution of the lipoma removal procedure are required in order to achieve a safe and radical result.<\/p>\n\n\n\n
