They may appear after an injury that causes a break in the continuity of the skin, regardless of the cause.

In other words, a keloid may appear after:

surgical procedure,

burn,

trauma and skin tearing,

tattoo,

even after acne.

Regarding keloids as a result of acne scars, this phenomenon is rarer and is usually due to cases of severe acne that was not treated promptly and properly, as well as to individuals with a predisposition to keloid formation.

What causes keloids?

In general, keloids are of unknown cause; however, most evidence suggests that they may be due to hereditary or hormonal factors. They usually begin to develop within the first month after the injury.

Most of the time, it is impossible to predict whether someone will develop a keloid after an injury. However, if someone develops a keloid on their body after an injury, they have a greater chance of developing a new keloid after a new injury.

What do keloids look like?

Keloids are cases of abnormal skin healing, resulting in scars that:

are raised, meaning they protrude above the normal skin like a relief

develop beyond the boundaries of the initial injury and are clearly larger in size

are red, with small blood vessels visible on their surface

cause intense and often distressing itching or even pain

do not subside over time, whether they are caused by acne or by any other factor.

In which areas of the body do keloids usually develop?

Keloids appear more often:

in the shoulder blade area,

on the sternum, and

on the ear cartilage and earlobes.

Who is more likely to develop a keloid?

Keloids appear more often in women than in men. The younger the person is, the greater the chance of keloid formation. The darker the skin tone, the greater the likelihood of developing a keloid.

In very simple terms, the people most likely to develop a keloid are young, dark-haired women with an injury on the shoulder blade, sternum or ear cartilage/earlobe.

What is the difference between a keloid and a hypertrophic scar?

Both keloids and hypertrophic scars are abnormal forms of wound healing. However:

Keloids extend beyond the boundaries of the injured skin, while hypertrophic scars remain strictly within the traumatic surface.

Keloids always cause itching, while hypertrophic scars do not.

Keloids are red or cherry-colored, with small blood vessels on their surface, while hypertrophic scars are not necessarily so.

What is the treatment for keloids?

It is true that the treatment of keloids is very difficult, given that the skin’s reaction is highly unpredictable in these cases.

What we know very well today is that surgical treatment of keloids should be avoided as the first treatment, because there is a high chance that this new skin injury may lead to the appearance of an even larger keloid, thus creating a vicious cycle.

The best treatment for keloids is performed by injecting a specialized corticosteroid substance into the scar. This is a substance that causes selective atrophy of the keloid, resulting in the raised projection subsiding and the scar flattening to the level of the normal skin. Usually, two or three sessions are required, with an interval of 4 to 6 weeks between them.

Apart from intralesional corticosteroid injection, special antineoplastic substances may also be injected into the scar, with very good results. With these treatments, the problem of scar projection is greatly improved or even completely resolved, but the red color usually does not disappear. This can be corrected with Laser treatment, so that we can ultimately achieve a very good scar quality.

Another treatment option for keloids is irradiation of the scar with therapeutic radiotherapy.

When is surgical removal of a keloid scar indicated?

In very rare cases, if a keloid is very large in size, part of the keloid may be surgically removed so that it becomes smaller and can then be treated with a more moderate method. In these cases, at the end of the procedure, a corticosteroid or antineoplastic substance is always injected into the scar in order to control the phenomenon and prevent the formation of a new keloid.

Keloids – Definition.
